Download your paperwork ahead of time, then arrive a few minutes early — our team will take care of the rest.

New patients at Logan University Health Centers should download and complete the patient forms below, then arrive about ten minutes early. Your first visit takes roughly an hour and includes a health history, examination and — when indicated — on-site imaging, followed by your plan of care. No referral is needed.

New patient questions

What should I bring to my first visit?
Bring a photo ID, your insurance card and your completed new patient forms. If you have had recent imaging or lab work related to your complaint, bring those reports as well.
How long does the first appointment take?
Plan for about an hour. Your first visit includes a health history, a physical examination and, when indicated, imaging, followed by a review of findings and a plan of care.
Do I need a referral to be seen?
No referral is needed. The Logan University Health Centers clinics are open to the public and you can request an appointment directly by phone or through this website.
Who will treat me?
A Logan student clinician provides your care under the direct supervision of a licensed Doctor of Chiropractic, who reviews your examination findings and approves your plan of care.
Do you accept insurance?
We accept most major insurance plans and submit insurance forms on your behalf. Call the clinic before your visit and our team will verify your coverage.
Do you offer X-rays and lab testing on site?
Yes. Radiology and laboratory diagnostics are available at our Chesterfield clinic, so most imaging and testing can be completed during your visit.
